This form is obtained electronically in advance of traveling, you can do this in English using this link https://www.spth.gob.es/ or via the app SpTH, available for Android and Apple users, but only in Spanish. After the form is completed, you will obtain a QR code, which must be presented upon arrival in Spain. You can enter and save the general data as soon as you have your flight details, but remember that you can only fill in the data with regards to the health questions in the window of 48 hrs before your planned travel date.
Upon arrival to Spain all passengers who come from abroad will be required to undertake a temperature test, a temperature equal to or greater than 37.5 ºC is detected. The temperature should be taken with thermometers without contact or by using thermographic cameras.
